Adds one paragraph to playbook 08, after "A fix that stops before its delivery": when a fix's delivery shows a second fault that keeps the issue's done-when from holding, it is taken into the same issue as a second pull request with its own review round only when it is in the same files; a fault in other code is an issue of its own.
Learned on issue 937 (postgres asked to retire the logins of running apps): mesh-catalog #335 was taken in, issue 943 was opened.
A playbook edit of a few lines: the operator's word on this pull request is its review (playbook 08 step 4). sh merge-check.sh passes locally, with the catalogue compared.

Delivery plan — builds nothing: the change touches no module of the mesh's graph
Transitions
2026-10-16 17:06 (new) → proposed (announced): novox/hq#386's head announced
2026-10-16 17:07 proposed → checked (checked): a verdict names this commit and the trunk's head as its trunk commit
2026-10-16 17:07 checked → ready (accepted): the gate passed or warned, and the repository's own check did not fail
2026-10-16 17:09 ready → published (merged): on the trunk its modules follow: merged there, and its walk opened — or nothing for a walk to move
2026-10-16 17:09 published → delivered (done): nothing for a walk to move
The commit's note under refs/notes/mesh-plan keeps every transition: git log --notes=mesh-plan.
