Issue 1781. Delivery group with novox/mesh-catalog feat/1781-milestone-records.

Design 47's milestone table: each issue in one row (the last row keeps an issue two rows named), R4a's range written out as numbers, row M1 for issue 1781, and the reasons recorded.
Design 53 §13 (milestones), with ADR 0493 in its decisions: a milestone is a record in the issue tracker; the seed's close rule matches the code (done is wontfix, resolved with every live fix delivered, or resolved with no live fix; closed only when none of its row's issues was refused).
