Issue 1281's live proof failed on 2026-10-20: every module key said 'no successful trunk build recorded its inputs', because IsTrunkBuild wanted state "built" and the controller stores an outcome with no state (mesh-controller internal/chain/records.go:57-59). Fixtures had agreed with the mistake.

• BuildRecord gains Failed; IsTrunkBuild = no state, not failed, source inputs, ref empty or main (1281/AC7).
• The build record fixture follows the controller's layout (state and ref removed, clone-URL repository), saying where it came from.

Group fix/1281-outcome-is-a-build with mesh-delivery (the indexer and the view take this SDK). Tried: build-1792450090609720774 pass.
